This is for a one-time virtual (phone call or Zoom) assessment with Jacqueline where you will give the details of your situation, and she will provide an overall prognosis and a recommendation on appropriate services. This service does NOT include a specific training plan or exercises.

After submitting the form below, an invoice will be emailed for the assessment (pre-paid) and it will be booked within 1-2 business days.
